We are conducting interviews for Ms. Cynthia Ellis as part of the study abroad program at Colorado State University. The purpose of this study is to enhance the adult education program at St. Ignatius. Your participation is voluntary. You will not be compensated and your grades will not be effected. Your signature below authorizes us to make notes or audio recording of your responses and to share these in individual and aggregate form with the faculty and staff at St. Ignatius. It also permits us to use photographs of the school in which you may appear. Data collection is confidential and will be stored in a secure location
